Casual Exercise Physiologist

At Milestone Health and Performance the majority of our clients(approx 70%) are those with Home Care Packages. Home Care Package(HCP) provide support to people aged over 65 to work on their health and ability to live in their own homes rather than move into an aged care facility. This funding is used for nursing, assistance with activities (such as transport, gardening, cleaning, etc), mobility aids(ramps, house mods, walking aids,etc) and Allied Health(Such as EP) to help them work on their ability to participate with their family, in the community and complete tasks around the house. This program not only saves the government money as compared to aged care admissions but also allows people to live where they would rather be and lead a healthier and happier life for a much longer period.
 
Our role in this is a vital part which can add important healthy years to someones life. The HCP field is a growing field and as time progresses the importance of exercise is further highlighted. Through exercise we can help to reduce the risk of falls, improve the ability to walk(with or without walking aids), improve confidence and safety with stair use, improve the ability to get up and down off the floor and being able to get out of a chair with ease. 
 
Our clientele value the support we provide and we generally see most of our clients between 2x per week and 1 x per month depending on funding and the need for support. As part of Milestone Health and Performance we are also committed to continually improving our service that we provide, keeping up to date with research, continually improving our service/exercise prescription and providing regular meetings to work on the above and questions.
 
The other 30% of our clientele consists mostly of NDIS clients- they are generally aged under 65 with a range of disabilities/medical conditions with the key aim to work on the management of their conditions, the ability to live independently, increase community participation and improve enjoyable exercise habits.

About the Role 

Milestone Health and Performance works with predominantly Home Care Package and NDIS clients in their own homes- assessing clients, recommending the best plan going forward and delivering it over time. Milestone Health and Performance is looking to add an EP to our team starting with 1-2 days per week with the potential to build to full time work. 
At Milestone Health we pride ourselves on our ability to assess and provide targeted, effective exercise to predominantly to older adults to help support them to continue to live in their own homes on a long term basis. We are always working on our service and prescription and are committed to providing the best service we can. Support and regular meetings working on exercise prescription, case studies and questions is included in the role. 

The aim of our service is to improve clients ADLs (Activities of Daily Living such as sit to stands, balance, walking and stair use), reduce the risk of falls and increase independence to allow clients to live in their own homes for as long as possible. The role involves undertaking home visits involving exercise prescription, some note taking and report writing (short progress reports for clients completed periodically and occasional NDIS reports). 

This position would suit someone who enjoys Work-Life balance, autonomy in their role (there is support available when required), the ability to work with regular clients over an extended period and who enjoys improving the quality of life of othersIf you are committed to learning, open to feedback and would like to be part a of team long term this position may be for you.

Home Visits

  • Regular Clientele
  • Consist of 30 mins of assessment/exercise with reassessment of ADLs completed every 6 weeks
  • Progress Reports(completed every 6 weeks) provide a short feedback loop with the ability to determine if treatment has been effective plus identifying areas to improve.
  • 5 weeks to prescribe exercise to help improve the outcomes of progress reports

  • Masters of Clinical Exercise Physiology
  • Police Check
  • Registration with ESSA 
  • Up to date COVID and flu vaccination as per aged care working requirements
